Aging and Adult Care
Carla Barnard, with Aging and Adult Care of Central Washington (AACCW), is the Health Home Care Coordinator Lead for Lincoln County. The Health Home program is free of charge for qualified Washington State Medicaid Insurance clients. Carla is available for appointments in your home or at the Davenport Senior Center on most Tuesdays & Thursdays.
The Health Program can help you:
– Find new doctors.
– Coordinate referrals to specialists.
– Medical supplies, dental, and vision services that may be available outside of the Apple Plan.
– Resources to help with housing assistance.
– Connect transportation services with Apple Health members.
– Mental Health Services.
– Resources to help with paying utility or other bills.
– Manage prescription drugs.
– Disease self-help programs, including weight management.
– Assist with State Medicaid Insurance and Social Security applications.
– Transition planning and much more!
